This school year, we are asking our families to help us limit the number of visitors at our school sites. This new process is for students participating in the traditional in-person setting as well as the Mid-Del Virtual Academy. Schools have mailed student verification forms to parents/guardians this week. If you did not receive your verification form in the mail, you can print a blank form to complete from the district website. If you do not have access to a printer, you may pick up a printed form at the Mid-Del Schools Administration building located at 7217 SE 15th Street in Midwest City. The forms will be located in the entryway.
If you have changes to your student’s information, please make the changes and email the document to firstname.lastname@example.org.
We are asking that you provide your proof of residency and verification forms via email by either taking a picture of your current utility bills or providing pdf copies of your bills. Please send these documents to email@example.com. When you email the bills, please make sure it shows the name, address, and date of service. Please send a July or August bill with the subject line containing: Student’s full name and current grade.
Please be reminded that you must provide the following information to your assigned school site to confirm residency information in August.
Parents that live in a house
2 Current utility bills (less than 30 days old)
Parents that live in an apartment
Current lease AND
1 current electric bill (less than 30 days old)
**If the student is on a resident affidavit, the utility bill isn’t in your name or you have some other special residency arrangement, you must go to the Central Enrollment Center to complete this process. For questions, please email firstname.lastname@example.org.
